Study Title: A Survey on Attitudes Towards Surgical Preparation
We want to understand how surgical performance and patient safety can be improved and we ask that you try to respond honestly and accurately. It is important to note that some of these questions may be considered sensitive in nature (e.g. around your surgical practice and level of training) and that you are free not to answer any specific question if you do not wish to do so.
This survey is divided into three parts and should take approximately 10 minutes.
There will be three prize-draws of £50, £30 and £20 in gift vouchers for participants selected by 31st August 2017. If you would like to take part in this please remember to leave your email address at the end. Your email address will be deleted from our records after this date.
Responses are anonymous and this survey complies with data-protection guidelines. The data will be handled by the project supervisor (Dr Mushtaq) and lead researcher (Dr Mir) on University of Leeds approved cloud computing services and encrypted laptop computers. The anonymised results from this study will be disseminated through conference presentations, publication in peer reviewed journals, compiled as a report, used in future projects, included on an institutional website, presented an electronic thesis and made available on an online data depository, but any one individual’s responses will not be able to be identified in such reports.
The data will be stored for 10 years. After this point, a review will be undertaken and University of Leeds Service IT managers will be consulted to establish whether it is still necessary to keep the data or whether it can be securely discarded.
You are free to withdraw at any time prior to submitting your answers by exiting this window. If you exit (by closing your browser) your answers will not be saved.
If you include your email address and decide to withdraw after submitting your answers you can do so by contacting email@example.com within 24 hours of submission (up until August 31st 2017 [whichever date is earliest]). If you do not include this information we will have no way of identifying and removing your responses.
