BOTA objection to the Trainee Fee and its recent rise

The JCST Phase 1 Review Report (link below) stated that 'The Trainee Fee risks alienating the next generation of surgeons, and any organisation which does not command the respect of its future members deserves to fail.' BOTA responded to this review (link below) and supported all 15 recommendations, including the simplification of ISCP, the rationalisation of surveys and visits, the incorporation of the Trainee Fee into College subscriptions, spreading the cost of the Trainee Fee amongst all members and fellows of the Royal Colleges, the agreement of the roles of GMC, JCST, SAC, Deaneries, Schools of Surgery and SSSTB and the avoidance of duplication between these bodies.

It is therefore disappointing to find that, not only have very few of these recommendations been acted upon, but the fee is to rise by £25, and is likely to rise further in the future, in the face of rising costs borne by the trainee. The reasoning behind the rise is contained within the JCST statement. BOTA calls for those in charge of surgical training to justify why trainees are used to address the financial shortfall, and instead of seeing trainees as a source of finance, involve them in the consultation process, and aim to provide training more effectively, and at a reduced cost.

Challenges we face

There are a number of challenges facing orthopaedic trainees

1. Current over-production of CCT holders
2. General lack of faith in the ability of central workforce planning to match supply with demand
3. Reluctance on the part of foundation trusts to appoint to substantive consultant posts at a time of uncertainty around commissioning
4. Increasing service demands as a result of demographics
5. Inability of the centre to require foundation trusts to appoint consultants
6. Agreement that there is no appetite for the creation of a specialist or sub-consultant grade

How do we think these issues should be tackled?

Request for survey participation

Dear All Surgical Consultants and Trainees,

I am a 3rd year medical student from Edinburgh University and am doing a project with Mr Mark Hughes, an academic neurosurgery trainee. We are interested in surgical simulation / training and specifically in the concept of surgeons 'warming up' prior to operating.

Elite sportspeople, musicians, and dancers perform in a high-stakes, high performance environment in which excellent psychomotor and cognitive skills must be carefully deployed. Such professionals routinely "warm-up": doing so improves final performance (1). Operative surgery is another skill-driven discipline demanding fine motor skills and appropriate cognitive arousal. In contrast with sport or art, however, errors in the operating theatre have critical consequences. And yet, surgeons do not routinely warm up.

Two randomized trials have assessed the pre-operative warm-up effect; one involving simulated tasks, the other laparoscopic surgery. Warm-up resulted in significantly improved cognitive and psychomotor performance, faster task completion, and decreased errors (2,3).

As part of a project that involves designing appropriate warm-up tasks we have developed a survey to gauge the opinions of working surgeons on this area. We are very aware that surgeons are frequently pestered by such surveys and hence it takes less than a minute to complete!

If you would have the time to complete the survey we would be very grateful. The URL for the online form is: http://surveys.questionpro.com/a/TakeSurvey?id=2934853

Thanks a lot,

James Loan and Mark Hughes
